What does a cresty neck in a horse mean?

A firm or enlarged cresty neck can be an early sign that a horse or pony is storing fat abnormally and may be at higher risk of insulin dysregulation and laminitis. It does not diagnose equine metabolic syndrome on its own, but it is a reason to assess body condition, spring pasture exposure and, where appropriate, speak with your veterinarian about testing.

For the good-doer, spring isn’t simply a welcome change in pasture. It can be the season when horses gain weight, a cresty neck becomes more pronounced and the risk of metabolic problems increases.

The good news is that recognising the warning signs early and putting a sensible management plan in place can make a significant difference.

Does a cresty neck mean equine metabolic syndrome?

One of the most useful visual clues that a horse may be predisposed to metabolic problems is the development of a cresty neck.

A firm, enlarged crest, which can be accompanied by fat deposits around the tail head, shoulders or behind the withers, can indicate abnormal fat accumulation and should prompt a closer look at the horse’s health and diet.

Not every cresty horse has equine metabolic syndrome (EMS), and a crest alone cannot diagnose EMS but it is recognised by vets as an important predictor of insulin dysregulation. Neck crest and abnormal regional fat deposition are warning signs, particularly in horses and ponies that are easy keepers or have previously experienced laminitis.

This is why late winter and early spring is the perfect time to assess your horse before the weight starts going on.

Don’t simply ask, “Does my horse look fat?” Look at where the fat is being stored, monitor body condition regularly and keep an eye on changes in the crest.

Why spring pasture increases laminitis risk

Insulin regulation matters

EMS is fundamentally associated with abnormalities in insulin regulation. Insulin is the metabolic hormone released to move sugar from the blood into storage or the muscles that need energy to function.  In insulin-dysregulated horses, the body doesn’t respond to insulin normally, causing problems with maintaining appropriate blood glucose levels.

For susceptible horses, periods of high pasture sugar and starch availability can place additional metabolic pressure on this system.

Importantly, managing an EMS-prone horse isn’t just about reducing calories. The quality and composition of the diet, pasture management, body condition and exercise all matter.

Reducing excessive intake of rapidly fermentable carbohydrates is an important part of management, but simply putting a horse on a very restricted diet for months isn’t the whole answer.

The goal should be to provide adequate nutrition while controlling excess energy and non-structural carbohydrate intake.

Exercise is part of the metabolic equation

Exercise is one of the most powerful tools available to horse owners.

Regular, appropriate exercise can help maintain muscle mass and support normal glucose and insulin metabolism. Working muscles can take up glucose through pathways that are not entirely dependent on insulin, making physical activity an important component of maintaining metabolic health.

That doesn’t necessarily mean intensive training. For many good doers, consistent movement is more important than occasional hard exercise.

Regular riding, groundwork, lunging, hill work, walking or simply increasing daily movement via track systems or placing food and water at opposite sides of the paddock can all contribute to a healthier management program. For a horse carrying excess weight, however, exercise should be introduced progressively and with veterinary guidance where there is a history or suspicion of laminitis.

Don’t let a reduced diet become a nutrient-deficient diet

One of the challenges of managing an overweight horse is balancing calorie restriction with nutritional adequacy.

When owners reduce hard feed, eliminate concentrates, soak hay or dramatically reduce forage, they may unintentionally reduce the intake of essential vitamins and minerals.

This is particularly important because many good doers need less energy but they still need adequate fibre, protein, vitamins, minerals and omega-3 fatty acids.

Hay can provide a valuable source of fibre, but its vitamin and mineral profile can vary considerably depending on pasture, soil, plant maturity, harvesting and storage. Most vitamin levels decline during hay storage, while both vitamins and minerals  can be leached during the hay soaking process.

Spring: act before the crest gets bigger

The best time to manage a good-doer is before the weight gain becomes obvious.

As spring pasture arrives, regularly monitor and record body condition, measure the crest, assess pasture intake, remove susceptible animals from high sugar pasture and review the complete diet. Consider whether your horse is receiving more energy than it needs—and whether reducing feed has also reduced its intake of essential nutrients.

For horses showing significant regional fat deposits, recurrent laminitis or other signs suggestive of insulin dysregulation, veterinary assessment and appropriate diagnostic testing should be considered REGARDLESS OF BODY CONDITION. It’s about providing the right nutrients in the right amounts, controlling excess sugar/starch intake, supporting normal insulin regulation and keeping the horse moving.
